摘要

目的

评估睫状肌麻痹下多光谱屈光地形图（MRT）在儿童视网膜屈光测量的可重复性和一致性。

方法

纳入8～10岁的学龄儿童，睫状肌麻痹后进行客观验光和眼轴长度测量；由同一检查者采用MRT重复进行两次视网膜屈光测量。视网膜屈光度和相对屈光度用视网膜偏差值（RDV）和相对视网膜偏差值（RRDV）表示。采用秩和检验，组内相关系数（ICC）和95%一致性极限（LoA）比较测量间的重复性和一致性。

结果

本研究共纳入146名儿童，平均年龄（9.22±0.61）岁。MRT与自动验光仪中央测量重复性ICC=0.94 （95%CI

［0.92， 0.96］）；周边视网膜偏差值（RDV）从中央到周边各区域重复性良好，最小ICC=0.93 （95%CI

［0.91， 0.95］），测量差异均无统计学意义（所有

＞0.05）。相对视网膜偏差值（RRDV）在中央30°内可重复性下降，最小ICC=0.37 （95%CI

［0.12， 0.54］），30°以外可重复性有所提高，最小ICC=0.87 （95%CI

［0.81， 0.90］）。RRDV所有参数连续两次测量结果差异均没有统计学意义（所有

0.05）。

结论

在低度屈光不正的学龄儿童中，采用MRT测量视网膜周边屈光在同一检查者连续两次测量值具有极佳的可重复性，中央屈光与客观电脑验光仪有良好且稳定的一致性。

Abstract

Objective

To evaluate the repeatability and agreement of multispectral refraction tomography （MRT， Thondar， Shenzhen， China） in measuring retinal refraction under cycloplegic condition in children.

Methods

Children aged 8 to 10 years were recruited. Objective refraction and axial length measurements were performed after sufficient paralysis. Retinal refraction was measured twice by one examiner using MRT. The retinal deviation value （RDV） and relative retinal deviation value （RRDV） were used to describe retinal refraction and relative retinal refraction. Wilcoxon Signed Rank Test， the intraclass correlation coefficient （ICC） and 95% limits of agreements （LoA） were used to evaluate the repeatability and agreement between two consecutive measurements and between MRT and autorefractor.

Results

A total of 146 children with mean age of 9.22±0.61 years were enrolled. Central refraction between auto refractor and MRT showed good agreement， with an ICC value of 0.94，（95%CI

［0.92， 0.96］）. RDV provided excellent repeatability， with lowest ICC value of 0.93 （95%CI= ［0.91， 0.95］）. The RRDV demonstrated descending ICC value within 30°eccentricity （lowest ICC

0.37， 95%CI= ［0.12， 0.54］ in RDV-15） and showed good repeatability beyond 30° （lowest ICC = 0.87 （95%CI= ［0.81， 0.90］）. Neither RDV nor RRDV showed statistically significant differences between measurements. （All

Conclusion

MRT provides repeatable measurements of retinal peripheral refraction under cycloplegic conditions in schoolchildren with mild refractive error.
