摘 要: 【目的】 回顾性分析偏心髋臼旋转截骨术(ERAO)治疗后的成人髋关节发育不良(DDH)患者再次行人工全髋关节置换术(THA)的方法和手术疗效。【方法】 对2002年11月至2014年6月在日本名古屋大学医学院附属医院接受THA术的24例DDH患者。这些患者均曾应用ERAO术进行治疗,术后随访或间隔102~269(197.9 ± 51.48)个月后,再次进行THA术。24例患者(25髋),其中男6例,女18例,平均年龄59.4岁,其中3例取下股骨头进行大块骨移植,塑形与髋臼上缘匹配后,进行松质骨螺钉固定,7例予以少量小碎骨打压植骨。术后严格随访,临床评估以Harris评分为标准。影像学评估则通过拍摄术前与术后骨盆前后位,患髋正侧位,观察假体位置、Sharp角等以及有无假体松动、脱位,感染,神经血管损伤等并发症。【结果】 本组病例平均随访时间12 ~ 145(93.7 ± 24.38)个月。术后无假体松动、脱位,感染,神经血管损伤等并发症。Harris评分由THA术前(52.1 ± 9.3)分提高至(85.5 ± 6.9)分,有显著性差异(P < 0.001);ERAO术前sharp角为47~58(54.4 ± 3.7)°,ERAO改善为(44.9 ± 2.8)°(P < 0.05),到THA术前sharp角平均为(45.8 ± 2.3)°,术后为(32.5 ± 3.1)°,与THA术前比较改善明显(P < 0.05)。【结论】 ERAO大大矫正了DDH患者髋臼畸形程度,增大了股骨头的覆盖面并纠正了异常的受力模式,延缓了股骨头坏死及骨性关节炎的进展。对其再次行THA术,术中大多无需骨块移植,术后康复时间短,疗效满意。
Abstract: 【Objective】 To investigate the clinical efficacy and technique of total hip arthroplasty (THA) for hip developmental dysplasia (DDH) after eccentric rotational acetabular osteotomy (ERAO) when treating developmental dysplasia of the hip. 【Methods】 24 cases of DDH patients were from the Affiliated Hospital of Nagoya University in Japan, underwent THA surgery from 2002 November to 2014 June. After follow -up 102 -269 months, on average 197.9 ± 51.48 months, since the patients had already underwent ERAO, then THA performed. 24 patients (25 hips), 6 males and 18 females, mean age was 59.4 years old. 3 cases of were underwent the large bone transplantation used the femoral head, through shaping and matching the acetabulum, then fixed by cancellous bone screws. And 7 cases were treated with amounts of small bone impaction. In the follow -up, harris hip score was defined as clinical evaluation standard, combined with or without infection, neurovascular injury and other complications. And radiographic assessment was based on the measurement and further comparison of pre - and post -operative Sharp angle, prosthesis loosening and dislocation,. 【Results】 After 12 ~ 145 months follow -up, on average 93.7 ± 24.38 months, Harris hip score improved from preoperative (52.1 ± 9.3) points to (85.5 ± 6.9) points, (P < 0.001); Meanwhile, the sharp angle was 47 ~ 58 degrees ERAO preoperatively, mean 54.4 ± 3.7 degrees, improving 44.9 ± 2.8 degrees after ERAO (P < 0.05). Through THA, the mean of sharp angle reduced from 45.8 ± 2.3 degrees to 32.5 ± 3.1 degrees (P < 0.05). 【 Conclusion】 ERAO greatly improved the acetabular deformity of DDH patients. increased the coverage of the femoral head and corrected the abnormal stress pattern, slowed the progression of osteonecrosis and osteoarthritis. And the later THA operation for these patients, mostly without intraoperative bone graft, postoperative recovery time were shorter, and the curative effect was satisfied.
